The easiest and most efficient way to fill out our first- and third-party trust applications is to use the two links bolded and highlighted below – this will complete the first stage of the process for you and send your form on confidentially to our Enrollment team.
If you would like to fill out the forms manually via Adobe Acrobat or on hard copy, use the following steps:
1) Find the form(s) related to the trust type and situation relevant to you.
2) Download the forms by clicking on the Download links.
3) Open the forms in Adobe Reader on desktop computer or smartphone.
4) Fill out the forms using the fillable fields provided in each document.
5) Return them to PLAN by attaching them as an email to [email protected].
This document, completed by the family, outlines the wishes of the donor(s) as to how s/he or they want the trust funds to be used to benefit a loved one.
A service provider must complete this form in order to receive payment from a Special Needs Pooled Trust account. PLAN uses the information from the form to report all payments to the IRS. For a Spanish version click here.
